Advantage’s cardiac disease management program enables us to deliver the most appropriate quality care for our patients while measuring outcomes and reducing patient need for urgent or emergent care. The program utilizes clinical tracks with a focus on patient self management of subtle and acute symptoms. The primary cardiac diagnoses for the Cardiac at Home program are congestive heart failure, heart transplant, myocardial infarction, coronary artery bypass graft, hypertension and cerebrovascular accident.
Our Diabetics program has been developed based on the national standards set by the American Diabetes Association (ADA). Patients and their families learn basic survival skills, as well as self-management techniques necessary to control diabetes and the acute and chronic complications associated with it. The Diabetics program incorporates the components of structure, process and outcome as established by the ADA.
Whether it’s a broken hip or elective knee surgery, we know that patients would ideally have orthopaedic rehabilitation in the comfort of their own homes. Our nurses and therapists are trained to provide education and treatment for hip and knee replacements, back injuries, arthritis, complex fractures, and other orthopedic problems.
We have developed a Surgical Recovery program designed to give back to patients their functional independence and provide maximum clinical outcomes.
Our Stroke Recovery Program enables us to deliver quality care for our patients who have recently suffered from stroke by educating them and their caregivers on all aspects of recovery, including prevention of future strokes and the recognition of early warning signs.
Our team of professionals comprehensively assess our patients for balance dysfunction and determine the root cause of a fall. To do so, we utilize standardized clinical testing tools, use the latest technology to treat pain and neuropathy, evaluate prescribed medications for balance-related side effects, and assess the patient’s home environment for potential problems. Thereafter, we work with the patient’s physician to develop and implement a customized treatment plan to correct balance dysfunction and treat injuries caused by previous falls.
We help patients with lung-related diagnoses learn to take better care of their own health. This innovative, home-based healthcare program takes a comprehensive approach to disease management, by providing patients with the education, skills, and tools they need to manage their own condition.
Our Psychiatric Relief Program offers collaborative, comprehensive psychiatric nursing services designed to prevent hospitalizations; increase access to mental health services; provide follow-up treatment; and assist families in maintaining community living and improving their quality of life.
