Reducing Hospitalization and Keeping Patients Healthy.
Advantage Home Healthcare keeps patients healthy and out of the hospital. We do this by first identifying “high risk” patients, such as with Congestive Heart Failure; Myocardial Infarction; Pneumonia; and COPD.

We then implement disease management programs and patient teaching guides:
  • Tele-monitoring, which allows early detection and decreases re-admittance. University of Chicago research revealed a 26% reduction in re-admissions when used in addition to regular nursing visits.
  • Complete Patient Teaching Guide(s) and Education, such as for CHF; Diabetes; COPD; and Wound Care (Vac Certified RN’s).
  • Promote Wellness Through Action, such as with Safe living environment; Diet; Lifestyle; and Knowing risk factors.
We become a partner in the patients’ care, through communication and accountability, which comes from: Point of Care Technology for our Nurses and Therapists; a Physician Portal for better outcomes through increased communication resulting in improved management of patient care; instant access to information from home care nurses; a simplified billing process for certification, recertification and care plan oversight; and a Patient Portal through which we encourage patient and family participation; allow the patient/family to interact with caregivers; and increased communication that empowers patients and their families.

Easier Recovery

Receiving home health care helps to reduce unavoidable readmissions to the hospital, and studies have shown that patients recuperating from illness, injury, or surgical procedures heal more quickly and more successfully when recovering at home versus in a medical facility.
